Activity Request Form

Need to use our facility?

To the extent that space is available, Capitol Hill Baptist Church allows groups and organizations to utilize our facility. Please complete the form below and submit it below. 

Once the event has been approved, you will be contacted by our Church Secretary to discuss the details and receive additional information. We ask that you please allow 7-10 days for approval of your request.

Event requests must be submitted at least two weeks in advance to be considered. If publicity
is requested, the form must be submitted at least six weeks in advance.
